3. ______________ Photocopy of passport (photo and
passport #).
4. ______________ Yellow copy of application form.
5. ______________ Copy of Travelers checks numbers.
6. ______________ Original prescription for eyeglasses
or contact lens.
7. ______________ Original prescription for any
medication to be taken.
8. ______________ List of any and all allergies
(food/medication, etc.)
9. ______________ List of any dietary problems or
limitations.
10. _____________ Drinking permission slip signed by
both parents (if allowed).
11. _____________ E-mail address (if available).
*All of the above handed in 3-4 weeks prior to
